Required Documents

You will need to bring the following to the pharmacy with you:

Completed Prevaccination Checklist Screening Form
Government ID
Insurance Card (Red, white and blue Medicare card is needed for those with Medicare)
CDC Vaccine Record Card (for booster vaccine)
